Clearance Instructions
SSOM clearance instructions for excused absence from class/clinicals:
SSOM and Trinity require mandatory COVID testing if you exhibit even one of these COVID symptoms: Fever, sore throat, cough, runny nose, nasal congestion, change to sense of taste or smell, shortness of breath, fatigue, body aches, headache, diarrhea, loss of appetite.
If you feel sick and will be missing class/clinicals:
Step 1 Immediately do a home COVID test.
Step 2: Write your name, date of birth, date and time of test on the cassette.
Step 3: Take a picture of the test cassette results and email it to wellnesscenter@luc.edu.
Step 4: After you have completed your test and sent it in, call Dial-a-Nurse 773 508-8883 to obtain an excuse note. If the nurse is on another call, please leave your name and phone number to receive a callback.
If your first test is negative, you will need to repeat the home COVID test 48 hours after your first test.
- Again, write your name, date of birth, date and time of the test on the cassette and email a photo to wellnesscenter@luc.edu.
- After you have completed and sent in your second COVID test, call Dial a Nurse 773 508-8883 to schedule a phone clearance appointment to return to class/clinicals.
- If you do not answer the phone when called, you will have to reschedule your appointment.
You can order free tests at COVID Home Tests | USPS. Home tests are also available for purchase at the front desk of the Wellness Center for $4.00.
* If you need a clearance to return to class/clinicals when the Wellness Center is closed, you can go to another provider to request a clearance note (primary care, urgent care, minute clinic, etc). That appointment would be through insurance and may require a copay or an office visit fee. This provider cannot be a relative.
